Transferring from another provider?

Once you've established your new patient enrollment, please email your child's immunization records to Support@ForneyPediatrics.net. If you are unable to email, you can also fax us at (469) 720 - 4068, or bring a copy during your initial visit. During your initial visit, you will also complete a short 'Medical Records Request' form. We will use this document to obtain any previous medical records from your prior provider

Immunization Policy

At Forney Pediatrics we believe, unless there are medical contraindications, that all children should be fully immunized according to the Center for Disease Control (CDC) and the American Academy of Pediatrics (AAP) recommended schedule. All new patients must be willing to fully comply with this recommendation. We believe this policy advocates for children and is in their best interest. This policy also decreases the chance of exposure, protecting our class of patients who either due to age (infants), or clinical composition, are unable to be vaccinated. If you are transferring from another practice, we ask you to submit a copy of your child’s immunization record prior to or during your child’s first visit. If your child is behind on their recommended vaccines, we are happy to assist you in getting them caught up.
